Applying for Affordable Housing and Related Forms
Applications for Affordable Housing in Niagara can be picked up in person at the NRH office, the Housing Help Centre, Hope House, Niagara Peninsula Homes, Project Share and at Ontario Works Offices in Fort Erie, Niagara Falls, Port Colborne, St. Catharines and Welland.
These forms can be viewed and saved to your computer or printed.

Application Form
This is the form you will need to complete in order to apply for affordable housing.
Read the instructions carefully before completing the application form. You will need to submit Canadian Birth Certificates or Immigration Documents for all household members.

Building Selection Form - Niagara Regional Housing, Co-ops, Non-Profits
This form will help you to choose from the many affordable housing options in Niagara. Once you have made your selections and have qualified for affordable housing, you will be placed on the Affordable Housing Waiting List.

Building Selection Form - Rent Supplement Program
This form lists additional affordable housing options that Niagara Regional Housing offers through our Rent Supplement Program. The Rent Supplement Program offers rent-geared-to-income assistance in private locations for a selected number of units at each location. Rent-geared-to-income is rent that is approximately 30% of a household’s total monthly income.

Request for Health and Safety Priority Status Form
If you feel your personal safety is significantly at risk or you have a serious health condition or terminal illness, you may be eligible for Health and Priority status. You will need to provide documentation from a designated professional.

Request for Homeless Status Form
If you are currently living without shelter, you may be eligible for Homeless status. You will need to provide documentation from a designated professional.

Request for Special Priority Status Form
If you have been abused by someone you live with, or have lived with, you may be eligible for Special Priority status. You will need to provide documentation from a designated professional.

Request for Modified Unit Form
If you have significant mobility issues and rely on a wheelchair as your main source of mobility, you may be eligible for a modified unit.

If you are currently a tenant of Niagara Regional Housing and would like to move to a different Niagara Regional Housing unit, complete the transfer and building selection forms:.

Transfer From Market to Rent Geared-to-Income: Request for Rent Geared-to-Income Subsidy Form
If you are currently a tenant/member and are paying market rent, you may be eligible to apply for a subsidy.
